Responsibilities
- Conduct thorough psychosocial assessments to evaluate clients' strengths, challenges, and support systems.
- Develop and implement individualized care plans in collaboration with clients, their families, and other members of the interdisciplinary team.
- Provide counseling, therapy, and crisis intervention services to clients experiencing mental health challenges, trauma, substance abuse, or other social difficulties.
- Offer guidance and support to clients in navigating social service systems, including assistance with accessing housing, healthcare, financial resources, and other community-based programs.
- Advocate for clients' rights and collaborate with community organizations, government agencies, and healthcare professionals to ensure appropriate services and resources are available.
- Maintain accurate and up-to-date electronic records of client assessments, progress notes, and treatment plans, ensuring compliance with legal and ethical standards.
- Collaborate with other social workers, psychologists, psychiatrists, nurses, and healthcare providers to ensure coordinated care and optimal client outcomes.
- Stay informed about current research, best practices, and legal regulations in the field of social work through ongoing professional development activities.
